Filled application forms shall be submitted in person to the
Admission Cell,Office of the Principal, KLR’s Lenora Institute of Dental Sciences, NH-16, Rajanagaram, Rajahmundry-533294
Andhra Pradesh on or before due date by post.
Applications unaccompanied by required certificates or applications with incomplete entries and ineligible applications shall stand rejected automatically. Please do not leave any column blank. Where information is NIL write NO / NIL.
Applications of the candidates, who furnish incorrect information, enclose false / incorrect certificate or approach through agents shall stand rejected automatically.
Application shall be filled in English by the candidate in his / her own handwriting.
The cover must be super scribed “For Admission into I year BDS course – 2017 - 2018”
Only for candidates who have downloaded the application form from the website must attach an DemandDraft Amount Rs.2,000 for Cat –A, Rs. 5,000 for Cat- B1 and or Rs. 7,000 for Cat- B2 along with thisapplication.

Documents to be submitted along with the application:
- Certified copy of the SSC or any equivalent examination showing date of birth and other particulars of the candidate
- Certified copy of Memorandum of Marks of the qualifying examination making eligible for admission (10 +2)
- EAMCET -2017 Hall Ticket and Rank Card for ascertaining merit
- GAP Certificate if 10+2 passed before 2017
- Community, Nativity Certificate
- Transfer Certificate of 10+2/ Equivalent.
- Photographs with Name & D.O.B.
- Bank Guarantees as per the Category of Admission
