Fill up the information in own handwriting.

Paste photographs (taken from the same negative). Do not staple the Photographs. Attest it from the Principal of your college / any Gazetted Officer.

Forward the Application Form (duly filled in) along with a Demand Draft of Rs. 2,850/- (Rupees two thousand eight hundred and fifty only) drawn on any nationalized Bank, in favour of 'The Registrar, Dr. D. Y. Patil Vidyapeeth" payable at Pune.

For NRI/FN/PIO : Form fee + Test fee is 200 US$. Candidates who have paid the processing fees during Indus End. Fare need not to pay again, but have to fill the new form which is on the website indicating their center for Entrace Test.

The last date of submission of application form (with late fee) is 07/05/2016

On Receipt of Application Form along with a DD of requisite amount, the Information Brochure will be sent at your given address by post / courier. The Information Brochure Serial Number shall be your Application Form Number for further correspondence if any
