Fill up the information in own handwriting. Paste photographs (taken from the same negative). Do not staple the Photographs. Attest it from the Principal of your college / any Gazetted Officer. Forward the Application Form (duly filled in) along with a Demand Draft of Rs. 2,600 (two thousand six hundred only) without late fee upto 30/04/2016 and Rs. 2, 850/- (Rupees two thousand eight hundred and fifty only) drawn on any nationalized Bank, in favour of 'The Registrar, Dr. D. Y. Patil Vidyapeeth" payable at Pune. Send the Application forms mentioning over the envelope "APPLICATION FORM FOR AICET (MBBS)" addressed to The Registrar,Dr. D. Y.
