Admission Process 2019

List of documents to be produced at the time of admission to I MBBS and MD/MS courses

(Arrange one set of original certificate and three sets of attested Xerox Copies separately in the order given for verification along with Scanned documents )

1) Aadhar Card

2) For Date of Birth (DOB) S.S.C. (or equivalent) passing certificate or School Leaving Certificate endorsing DOB/ Birth Certificate with Name of Candidate.

3) Online Submission Form (Latest)

4) Admit card of NEET-UG/NEET-PG for PG program

5) Mark-sheet of NEET-UG/NEET-PG for PG program

6) Nationality Certificate issued by District Magistrate/Addl. District Magistrate or Metropolitan Magistrate (Competent Authority for issue of such certificate)/valid Indian passport/School Leaving Certificate of HSSC /12 Std. indicating the nationality of the candidate as 'Indian'

7) Domicile Certificate of Candidates from Maharashtra State issued by District Magistrate / Metropolitan  Magistrate/ Addl. District Magistrate or Competent Authority for issue of such certificate.

8) H.S.S.C. (or equivalent) examination mark-sheet.

9) S.S.C. (or equivalent) passing certificate

10) H.S.S.C. School Leaving Certificate

11) Migration Certificate issued by the respective Board/University applicable to After passing H.S.S.C/ Qualifying examination, students who have joined any other course for period of more than six months.

12) Gap Affidavit on Rs. 100/- stamp paper by the student duly certified by Executive Magistrate   

13) In case duplicate certificates are submitted, then original copy of F.I.R. and Affidavit towards loss of original certificates on Rs. 100/- stamp paper by the student duly certified by Executive Magistrate

14) Medical Fitness Certificate on letter head .

15) Proof of permanent residential address (Xerox Aadhar card / Passport / BSNL telephone bill /  electricity bill)

16) Parent's Pan Card (Xerox).

17) Identical Passport size photographs 

19) Caste Certificate

20) Caste Validity Certificate

21) Non Creamy Layer Certificate Valid up to 31/03/20 (VJ, NT1, NT2, NT3 & OBC including SBC)

22) Income Certificate mentioning total Income of family

23) Student's bank passbook Xerox with bank details.

24) Ration Card Xerox (OBC, SBC, VJ, NT)
