5. Two copies self-attested photocopy of admit card of Madhyamik/equivalent examination

6. Original school leaving certificate from the head of the institution last attended

7. Original character certificate from the head of the institution last attended

8. Self-attested photocopy of blood group report by the pathological center and eye sight certificate required (color blindness and spectacle details)

9. Two copies self-attested photocopy of SC/ ST/ OBC certificate (for reservation category candidates only)

10. Two copies self-attested photocopy of physically challenged certificate (for PH candidates only)
