Duly filled application form with all relevant documents and DD for Rs. 1,000/- in favour of "KSSPCMA Payable at chalakudy, Thrissur" and Non-Resident Indians are to submit DD for Rs. 1,250/- drawn in favour of the General Secretary, Ezhuthachan College of PharmaceuticalSciences, payable at Trivandrum should reach college office

on or before last date

• Certificate in proof of Age
• Mark List of the qualifying examination and 10th & 12th certificate
• Transfer Certificate & Conduct Certificate
• Pass Certificate
• Income Certificate
• Migration Certificate
• Certificate in respect of reservation under SC/ST/OBC
• Self addressed envelop (25 x 12 cm size) with India Postal Stamp sufficient for sending the selection memo by registered post shall also be attached.
