Post filled application form fee along with the following documents:

• Photocopy of mark sheets of the qualifying examination duly attested by the Gazetted Officer.
• Photocopy of Transfer Certificate and Conduct Certificate from the Head of the Institution last attended duly attested by the Gazetted Officer.
• Self addressed envelope with postal stamps worth Rs.10 affixed.
