- Photocopy of mark sheets of the qualifying examination duly attested by the Gazetted Officer.
- Photocopy of Transfer Certificate and Conduct Certificate from the Head of the Institution last attended duly attested by the Gazetted Officer.
- Self addressed envelope with postal stamps worth Rs.10 affixed.
