Admission to the BPT and MPT program is based on marks obtained in University Entrance Test.

Affix recent self attested passport size color photograph
Attach required documents along with filled up form
Applications are also available at the Administrative Office of Yenepoya (Deemed to be University).
Duly filled in applications may be sent to the
Principal Yenepoya Physiotherapy CollegeUniversity Road, DeralakatteMangaluru – 575018along with a DD for a sum of Rs. 1000/- for PG Program, Rs. 500/- for UG Program in favour of Yenepoya (Deemed to be University) payable at Mangaluru, towards Entrance Test Fee.
List of Documents to be produced:
While submitting the application for admission to the course (only Photocopies to be produced)
1. Mark cards (for all the years) of the qualifying examination
2. 10th Pass certificate (for proof of date of birth/birth certificate
3. 10+2 certificate.
4. Degree/Provisional Degree Certificate issued by the University.
5. Registration Certificate issued by the Council – if applicable
6. Community certificate (for minority candidates)
7. Domicile certificate/Nativity Certificate
8. NEET Score Card
9. SC/ST/OBC certificate together with income certificate
10. Medical/Physical Fitness certificate from a Reg. Med. Practitioner
11. DD for Rs. 1000/- for PG programs, Rs. 500/- For UG Program in favour of “Yenepoya University” payable at Mangalore towards application/registration fee.
At the time of counseling /Selection/Interview, All the documents in original to be produced for verification
