Students seeking admission, can obtain the application form and prospectus from the College or Padmavathi Information Centre, ADOOR ( Pathanamthitta (Dist), Kerala) in person by paying Rs. 250/-.

Students can also obtain the application and prospectus by post by sending D.D. of Rs. 300/- in favour of Sri Padmavathi College of Nursing, Guntakal payable at Guntakal.

Copy of certificates to be enclosed with the application :

• X Class certificate cum marks.
• Intermediate/ HSC/ PDC/ +2/ Vocational Certificate with mark list
• Transfer Certificate.
• Caste Certificate of the SC, ST and BC ‘C’ students.
• Two Copies of passport size photographs.
• Two self addressed and stamped envelops (Rs. 8/- each).
• Migration Certificate from students of other states.
• Certificates of Physical fitness from an Asst. civil surgeon.
• P.B.B.Sc / B.Sc Certificate (for M.Sc)
• GNM Certificate (For P.B.B.Sc)
