Interested candidate can collect prospectus and application form at institute admission office on payment of Rs. 500/-.

If application form is downloaded from website, pay cost of form Rs. 500/- by NEFT.
Filled application form along with self attested photo copies of certificates should reach
Director, ICMR- National Institute of Occupational Health, Meghani Nagar, Ahmedabad-380016, Gujrat, India.
Candidate should write " APPLICATION FORM ADMISSION TO AFIH 2021" on top of the envelope.
The Scanned Copy of application with attached documents and NEFT transation receipt shall be sent to afihnion&gmail.com
List of documents to be attached (selt attested photocopies)
- MBBS Degree
- Intership certificate issued by Medical College
- MCI/ State medical Council Registration certificate
- SC/ST/OPC/PH certificate (if applicatble)
- Experience certificate
- Sponsorship Certificate/ No Objection certificate in Original, if employed
- Name change certificate if there is name change
- NEFT transaction receipt
