1. Allotment Letter through UP-SEE/ Admit card & rank letter
2. Deposit Money receipt of Rs. 10,000/4,000 at counseling
3. Caution money deposit receipt of Rs. 5000/- at counseling
4. Address proof/ Domicile
5. Transfer Certificate/ Migration certificate (Original)
6. Attested copies of H.S./Intermediate/ Graduate or any other equivalent examination certificates and marksheets ( two sets)
7. Five Passport size color latest photographs
8. Medical Certificate
9. Income Certificate
10. Caste Certificate (SC/ST/OBC)
11. Gap Affidavit (Original if any)
12. 5 Self addressed envelopes with Rs. 5.00/- stamp on each.
