1. Download, Print & fill Application Form.
2. Draw a DD for Rs 500/- in favour of The Principal, JSS Institute of Speech and Hearing, Dharwad.
3. Send Application along with the DD and related documents to below address

• SSLC Marks Card & T.C for age proof
• Second PU or 12th Std equivalent Marks Card
• Migration Certificate for non-Karnataka students
