Admission Process
Applicant may collect application form and prospectus from the institute admission office.
Fill application form.
Attach required documents along with filled up form.
Application, complete in all respects, must be sent under Registered A.D. cover addressed to
DIRECTOR, HARYANA SCHOOL OF NURSING, ELLENABAD.
Application received after the specified date and incomplete in any respect will be rejected.
The application on the prescribed form must be attached with following documents :
1. Attested copies of the educational qualification etc. with mark sheets
2. Attested copies of all testimonials from the Head of the Educational School last attended.
3. Three recent passport size photographs are needed and one photograph should be duly pasted on the application form. (Not stapled)
4. A certificate in the form of an undertaking from parent/guardian promising to meet all financial obligations of the candidate during the training.
5. An eligibility/equivalency certificate is needed, (if required)
6. Medical Fitness Certificate. Doctor's team will be arranged by Haryana School of Nursing, Ellenabad.
7. List of visitors with photographs of the visitors and specimen signatures.
Note: Medical Examination for Medical Fitness shall be concluded by College hospital doctor prior to interview. A payment of Rs. 300 will be charged for this and only those who are found to be medically fit will be considered.
