Candidates seeking admission to the Master of Social Work – M.S.W., course should fill up the application form in a format given below and send it to -

along with a demand draft for Rs. 200/- being the registration fee and Rs. 300/- towards the cost of the application which may be downloaded and filled in. Altogether a demand draft of Rs. 500/- (Rs. 200 as registration fee and Rs.300/- as application fee) drawn in favour of Dr. M.V. Shetty Memorial Trust, Mangalore , payable at Mangalore may be sent along with the filled in application form. 

Attested copies of the following certificates should be sent along with the application form. 

• S.S.L.C. Certificate (10th)
• Marks cards of the qualifying examination (degree)
• Provisional Pass certificate issued by the university
• Transfer certificate issued by the College/School
• Conduct certificate issued by the Principal
• Date of birth in Indian Almanac (A.D.)
