 v) Punjab Residence Certificate

 vi) Demand Draft of Rs. 3000/- (in original)

vii) Self Undertaking (not availed any Residence benefit in any other state)

viii) Self Undertaking of Gap year (if any)

ix) Matric or equivalent certificate for Date of Birth

To be accompanied by a demand draft of Rs 3000/-in favour of "The Registrar, Baba Farid Universityof Health Sciences" payable at "Faridkot"

The prospectus and admission application form available on University website, can be downloaded and duly filled form submitted along with admission application fees as given below.

 MSc Courses application fee : Rs 2000 /- 

The admission application fee must be submitted in the form of Demand Draft in favor Registrar, Baba Farid University of Health Sciences, Payable at Faridkot.

The following self attested photocopies of documents are to be submitted with the ADMISSION APPLICATION FORM

i) Passport size colour photograph, self signed by the candidate.

ii) Self attested photocopy of Matric or equivalent certificate for Date of Birth.

iii) Self attested photocopy of character certificate from Institution last attended.

iv) Self attested photocopy of Detail Marks card and Degree of Qualifying exam.

v) Self attested photocopy of certificate in support of claim under reserved category as per the specimen given in Prospectus.

vi) Undertaking by the candidate for Gap year in original, if any.

vii) Undertaking from parent/Guardian.
