• Rs. 200/- for ST, SC, PC and Female candidates.

• Upload scanned image of recent color photograph of the applicant (100 KB size)
• Upload scanned image of Full signature of the applicant (30 KB size)
• Upload scanned image of Left thumb Impression of the applicant (50 KB size)

Document verification original document to be brought on the day of reporting at the nodal center for counseling (NCC)

Candidate should required to produce the following original documents as applicable at the time of reporting the NCC.

1- Pass certificate of the High School Certificate (HSE) examination awarded by BSE, Odisha/ 10th standard examination certificate declared equivalent by BSE, Odisha in support of qualifying examination.

2- Mark sheet of HSE examination awarded by BSE, Odisha /10th standard examination declared equivalent by BSE, Odisha securing minimum marks as per the eligibility criteria.

3- Caste certificate (SC/ST) from the competent authority issued in the Frofavma in Form-1/ the prescribed format of the competent authority.

4- Green Card & Parent issued by concerned CDMO (incase of candidate calming seats under Green Card category)

5- Certificate in Form –II from the competent authority for reservation Ex- Serviceman quota.

6- Z-Domicile candidates have to produce the following certificate :

a- Residential certificate from the competent authority of their state.

7- Mark seat of HSE examination /10th standard examination of the institute located in their state, declare equivalent by BSE , Odisha securing minimum marks as per the eligibility criteria

8- Candidates have to produce the following certificates

a- Residential Certificate from the Revenue Officer not billow the Rank of Tahasildar that he/she belongs to outlying Oriya speaking tracts in Form-III (Part-I) .

b- Mark seat of ME or HSE examination in support of having oriya as a subject of at ME or HSE examination in from –III (Part-II).

c- Employers certificate in from –IV as applicable.

d- Permanent residency certificate (in case of reservation category / lateral admission) in from –v as applicable.

e- ITI pass /10 +2 Science (PCM) / + 2 Vocational pass certificate and Mark seat issued by SCTE & VT , Odisha/ CHSE/ 12 age standard examination declare equivalent by CHSE. Odisha respectively.

f- Annual income certificate as appended in form –VII / in the format from the competent authority in case of candidate claiming seats under TFW category not earlier then April 2016.
