Take print out of the form and fill it properly.

Dully filled up application form should be submitted the university admission office along with required documents and application fee in the form of DD drawn in favour of “" A Shama Rao Foundation"  payable at Mangalore.

Save the registration number and one copy of duly filled form for future reference.

 Following Original Documents are required at the time of admission

1. 12th standard equivalent marks card / pass certificate – Original + 3 attested photocopies.

2. 10th standard pass certificate for proof of date of birth – Original + 3 attested photocopies.

3. Transfer certificate from the institution last attended – Original + 3 attested photocopies.

4. Migration certificate from the University / Board – original + 2 copies

5. Conduct certificate from the institution last attended – Original only.

6. Physical fitness certificate from any Regd. Medical Practitioner – original only

7. Blood group certificate – original only

8. Passport Size Photographs: 10

9. Foreign nationals selected for admission will have to comply with the following Additional requirements

10.  No-Objection Certificate from the Government of India.

11. Student visa for the period of study at the institution.

12. Valid Passport.

13. Score card of Exam

14. Any other document
