The Application Form and Prospectus for the B.C.A. can be obtained from the Office of Admissions on payment of Rs.300/- in cash or through demand draft drawn in favour of "MITS SCHOOL OF BIOTECHNOLOGY" payable at Bhubaneswar. 

Applicants are requested to submit their filled in Application forms along with self attested copies of the following documents:

• CLC and Conduct Certificate
