Fill up the information in own handwriting. Paste photographs (taken from the same negative). Do not staple the Photographs. Forward the Application Form (duly filled in) along with a Demand Draft of Rs. 1,350/- (Rupees one thousand three hundred fifty only) drawn on any nationalized Bank, in favour of "The Registrar, Dr. D. Y. Patil Vidyapeeth" payable at Pune. The last date of form submission is Without late fee Wednesday, 15/06/2016 and With late fee of Rs. 250/- (Rs. 1600/-) upto 22/06/2016 (Wednesday). Send the Application forms mentioning over the envelope "APPLICATION FORM FOR AIAHCET-2016 (BPT)" addressed to The Registrar, Dr. D. Y.
